A simple pre-visit workflow
Staff generate an intake link from an appointment. The patient opens the mobile-friendly form, completes the active practice template, and submits it before the visit. The submission remains connected to the appointment and customer record.
Expiring, single-use links
Each link stores only a hash of its random token. It expires after 72 hours and becomes unavailable after submission, reducing the risk of an old link being reused.
Keep staff in the workspace
Staff can view intake submissions from the appointment or customer record. The form template supports practical fields such as text, select, boolean and date values, so practices can start with a focused history rather than a long generic questionnaire.
Use booking and intake together
Pair public online booking with pre-visit intake to reduce front-desk data entry. Online appointments and their intake submissions remain inside the tenant workspace and follow existing access controls.
